Palliative Care

Our community based palliative care program provides specialized medical care for individuals living with serious , chronic illnesses. The focus on relief from symptoms, pain, and stress. Care can be provided in the patient's home or in an Assisted Living Facility. Our palliative care program is designed to improve quality of life for both patients and their families by offering an extra layer of support in coordination with the patient’s existing healthcare providers. Care is delivered by an interdisciplinary team that may include, nurse practitioners, nurses, social workers, and other professionals who work closely with patients and families to address physical, emotional, and spiritual needs, helping them navigate complex medical decisions and maintain comfort and dignity.​​
